Can sites please weigh in as to whether de-identified clinical notes are ever provided to researchers? If so, answers to any/all of the following would be great to know: 1. Does your site use any standard processes to de-d clinical notes? If so, can you please describe? 2. Are other attributes like provider, encounter info, etc. available along with the actual notes? 3. Are certain types of clinical notes not captured or not made available to researchers (e.g., phone consults that do not result in a provider referral, etc.)? 4.
