Updated patch with even more migration and many clean-ups
This patch fixes the issues reported and also cleans up much of the unused
code. We still need to fix the episode list context menu / how to handle
episodes. With the new database, we can have more/better status than before.

We should allow modifying the "is_new" flag when "is_downloaded" is false or
both "is_downloaded" and "is_deleted" are true.
We should allow modifying the "is_deleted" flag only when both "is_downloaded"
and "is_deleted" have the same value and "is_new" is false.
Downloading should be possible when "is_downloaded" is false or both
"is_downloaded" and "is_deleted" are true.
Deleting should be possible when "is_downloaded" is true and "is_deleted" is
false.
Does that sound reasonable?
