Disable TreeModel cache for channels

TreeModel for channels is cached in memory and is frequently rebuilt, for all
channels at once. This happens (1) when config is changed, (2) when a file was
downloaded or cancelled, (3) after sync to ipod. This gets slow when there are
many channels. Even more so because this now involves fs lookup to detect
manually deleted episodes and mark them so in SQLite. This is quite a task,
especially with many channels and high gl.max_episodes_per_feed values. I
don't know about the memory footprint, but it might be significant, too.

I removed podcastChannel.__tree_model and forced model updates; currently, it
is rebuilt when a channel is activated
(self.treeAvailable.set_model(self.active_channel.tree_model) in gui.py and
only there).

Cons: displaying the list of episodes for a channel can be a little slower than
it used to be when it was cached.

Pros: settings are saved faster, downloads are started/cancelled much faster,
gPodder is more responsive and "gets to think for a while" less often.

Please test. There is room for improvements, but only if this change is
generally accepted.
