Rick Jacoby gave more information on the question: Update: I found apache/wsgi/django was using libcairo.so.2 from /usr/lib64 and NOT the cairo lib from my LD_LIBRARY_PATH. I was able to replicate this problem and fix it on our dev server since I have sudo there - I deleted conflicting cairo. But I do not have sudo on production server in dmz. So I need to figure out how to override this behavior.
