Previously the menu items were consistently added in the same order.
Code changes (I don't recall if it was in Firefox or Greasemonkey)
then caused the order in which the menu items were added to reverse,
after which a patch was added to Greasemonkey to change the order back
to the original behaviour. That is most likely why you have seen
multiple changes recently AFAIK.

It should remain consistent from now, and any changes will likely be
considered a bug afaik :)
