Maybe you added it to the Add-on bar and removed that? Press "Ctrl + /"
to show and hide the Add-on bar.
If it's not there, did you add the icon to the toolbar? Rightclick on an
empty place in the toolbar, click on "Customize..." (last item) and drag
& drop the icon to the toolbar.
