Never thought of that.

It was a corrupt permissions.sqlite issue.

I followed the step through, based on your idea, at
http://kb.mozillazine.org/Standard_diagnostic_%28Firefox%29#To_fix_the_problem_in_the_original_profile

Step by step and I found it.

Seems that your Firefox profile is broken.

Try to create a new profile (using -profilemanager option when starting FF) and install GM there to verify if it is working on your system.
