The command `\Vbarsmall` doesn't exist in v4.0. One of the new features
for 4.0 was a significant redesign of the barred letters so that they
have a more consistent look with the font you are using. In v3.0 and
prior, the whole glyph (bar and letter) was precomposed in gresym.ttf.
This meant that if you changed the document font, the letter component
of the barred letter didn't change to match. In v4.0 only the naked
bars are defined in greextra.ttf and these are then composed with the
letter from the document font to achieve the final glyph. Defaults for
`\Vbar`, `\Abar`, and `\Rbar` are defined based on the Linux Libertine
font, but if you're using a different font or want access to the bar
variants you'll need to define them yourself in order to make sure that
they look right.
Take a look at the 4.0 documentation (in particular the section 2.2.8
Barred Letters) for more details on the new system and how to define
your barred letters to achieve the look you want.
