A *much* more flexible tool is xindy, written in clisp.  Since it is
part of texlive 2012, it is available ready-to-run for many platforms.

Unfortunately, noone has written a proper style file for roff, AFAIK.
I believe it shouldn't be too difficult, and its author, Joachim
Schrod, can certainly help.
