Forgot to mention, I am using stock groff as of (K)ubuntu 13.04 GNU groff version 1.22.1 which comes with mom bundled and I built the referenced PDF https://github.com/the42/makeindex/blob/master/troff/mom/mom-pdf.mom.pdf?raw=true using the command pdfmom -R -U -mindex mom-pdf.mom > mom-pdf.mom.pdf
