And here's the before-and-after on how Kernighan & Cherry's eqn user's
guide renders the problematic spots, with Tadziu's size fix and my
ham-handed italic corrections.

I'm still not thrilled with the nested superscripts case--it's better
but it still irks me a little.  At the same time I'm not yet convinced
that's a problem with the italic corrections; it may be something than
eqn itself should cope with.

What do y'all think?
