You have to specially request the role  using voms-proxy-init
otherwise you will not get it.
In this case it would be

voms-proxy-init -voms test_vo_mysql:/test_vo_mysql/Role=VO-Admin

otherwise he will have no role.

The role-name of the user is not getting reflected in the above command.
Is the VO credential correctly formed?
