I think places like Access technology institute and American printing house both sell material on using MS office 2007 with a screen reader, and for the Word forms, you probably need something along those lines.
A Word form is nothing but a document with fields inserted, and at the end of the process, you choose to "protect" the form so that the portions which aren't fields cannot be edited. Unfortunately, these often are not very accessible, since you can't move the cursor around to judge the instructions or what information is desired; it relies on the user using the mouse cursor.
