Will that affect any of the normal sounds that we depend on? This loss of sound is a tricky one and not always consistent. I can sometimes turn on NVDA and it speaks and other times it won't work. Sometimes when I do turn on NVDA DECTALK express speaks a little, but only a few keys, not the whole thing comes back. Very strange!

You can try this to disable the sound enhancements that come with Realtek cards or chips. This is in Windows 7.
1. Open the Sound dialogue box through Control Panel.
2. Select "speakers (realtek)" in the list of devices, and then click properties.
3. Ctrl-tab to the enhancements tab.
4. Check "disable all enhancements".
5. Click ok.
6. Click ok again.

sound enhancements turned on by default that wreak havoc with
everything. So first try to find that in device manager or control panel

couldn't get without RealTek onboard, do yourself a big favor and
disable it in bios; this will require sighted help, then grab a PCI,
PCIE, or USB sound card and life will be a lot easier.
