With one of the programs running that exhibits the problem open the Window-Eyes control panel.
Open Keyboard, arrow down to voice.
Tab once to the scope button. If you want the same in all programs make sure it's set to Global instead of Program.
But regardless, Tab down again and you'll be in the list of settings.
Make sure it's set the way you like it.
Control-S Save.
If this doesn't solve the problem try turning apps off to make sure it isn't an errant app.
Alt-A Apps.
Enter on App management, App status, and Off.
If that solves your problem then an app is making changes.
