If you trying to set up commanders for an app.
First go to the voiceover utility with vo f 8.
Next go to commanders by pressing command 8.
 Next tab to the add button and then press vo space bar on it.
Next type the letter you are going to apply to the app.
Next vo right arrow to the command menu button.
Next  press vo space bar on the command menu button.
Next go to custom commands and then press the right arrow on it.
Next if you are trying to run a apple script you can choose run apple script.
After that vo right arrow to the list view table
..
If you are trying to set up the commander to open a app choose the open 
application option and then vo right arrow to the list view table.
Next use the up and down arrow keys or letter navigation to  locate the app you 
want to apply the commander to and then press command o to apply the commander.
