Firefox, this is a known issue.  Also happens in Thunderbird.

I have had a few instances where I have had to toggle browse mode off, 
even though there isn't a buffer. Also, on a slightly related note, has 
anyone else been getting the verbocity elements of a webpage, e.g. 
number of headings, links, landmarks etc. Read to them 3 or 4 times in a 
row in rapid succession? Noticed this on firefox but can't remember 
about ie. Is this a documented bug?
