NoScript might be getting in the way--assuming you're using it. Try either the allow scripts globally (temporarily, turn it back on after filling out the form) or maybe the allow all this page or temporarily allow all this page option. When I used Firefox on Amazon, the departments combo box disappeared until I allowed scripts globally.
