AFAIK, when cables are laid (such as when virgin come and dig up the road) they'll lay a plastic conduit with a coaxial cable in, that has plenty of room for a fibre optic cable next to it. This means that when it comes to installing the FO, they just open up the inspection chambers and feed it through the conduits, with no further digging up required.
