I don't want to scare you, but hamsters usually only live 2-3 years. So it is most likely
just old age. Milky eyes is most likely cataract which is very common in older mammals, It is really no big loss for your hamster pal, as he has never had very good vision. Try to hold him and say your good byes. If your lucky he'll go peacefully sleeping. Try not to over exert him.
