The problem is the inability to associate ancillary orders and other professional fees with encounters for ambulatory and inpatient episodes of care.  In other words, to find them all in one place (module).  If any one has a solution for that I'm sure there are many who would be interested including myself.
 
The VA has been contracting out coding services for years, however since the inception of the VA Revenue Cycle Improvement Initiative, they have been looking for other solutions.
 
There are a couple of commercial VistA billing solutions that are currently in use but they are not available with FOIA.  One, Quadramed, has integrated it products into VistA at many VA medical centers, and it encodes and groups data as patient records are processed, then incorporates the coding assignments directly back into the patient records.
 
Once you've done this, it's a simple matter to build an interface to just about any billing system.  But I think the cost of such a system is probably overwhelming.
