To my knowledge, DHCP/VISTA has always been uppercase only... throughout the
whole 20+ years. It's even likely that the first systems were only capable of
uppercase ;-)

I would be fairly confident that "someone local" did convert your patient file
to allow both. I've had that tendency for years myself. I think we lose a
little information by not having mixed case.
