In the RFI the government called for use of open, non-proprietary standards and implementations for the NHIN, and that means open source can be a logical choice for many if the open source products are good enough. This RFI is government's blatant admission that open, non-proprietary standards are in the best interest of health care. The government's identification of VistA as a viable example is a blatant admission that open source software itself is to play a major role. Much to the credit of this community and others, open source set a standard of licensing by which the government realized it could build an NHIN to serve the healthcare community.

This is definitely not the case. They actually *dictate* the design of a lot of healthcare software applications. They might not be companies like McKesson or IDX, but they do certainly make general health care systems infrastructure - especially that which is suitable for the national health information network (NHIN). They have a HUGE stake in healthcare.
The RFI, and the recognition by these three companies that open, non-proprietary standards will be the basis for software development does a couple of things. First, it further levels the playing field for open source software - given a non-proprietary open standard who makes the better product? And secondly, from my perspective the RFI is a "stake in the sand" for government and may very well achieve what many have suggested - that use of open standards mandated by the government will help stimulate open source adoption.
Also notice, that in the list of existing programs related to the NHIN, VistA is mentioned *several* times. See http://www.hhs.gov/healthit/federalprojectlist.html

The only way integration will happen is for customers to DEMAND it. So far the market has not been able to effect this....maybe it needs to be legislated...like the FAA does with aircraft maintenance standards etc. All it would take would be a scientific and credible study that links hospital error to a lack of integration and the associated mortality rate and things would change quickly IMHO. The evidence is there....
