I find the study itself a very good read, cleverly structured and perfectly   
timed. I am familiar with the TDS system used in the study hospital, it's an   
old war horse that has apparently been replaced last year in the study   
hospital. But the article would make a pretty good plot for an episode of   
ER.   
   
The authors' recommendations include this: "Plan for continuous revisions   
and quality improvement, recognizing that all changes generate new error   
risks." Seems to me that points us toward a key strength of Vista, the   
integration of users into ongoing design and maintance of the system.  
Translating that culture to the outpatient settings that interest me is a  
major problem.
