The difference was in the PACKAGE PREFIX field value. I do not remember what was the PACKAGE PREFIX value for the "ONCE" entry (I am not at my VistA system). I set the value for the "ONE TIME" entry PACKAGE PREFIX to "LR" and it worked. Not sure if changing the "ONCE" entry package prefix value would break anything, but did not want to take chances there.
