So, you probably want to use makemkv to extract a bit-perfect mkv file. Then
run that into handbrake.  Use one of the presets on the left.  If you use
high profile it will take a long time for a 1080p rip, but that is best if
you want a high quality rip.  rip it to a m4v with h.264 inside.  You have
to play around a bit if you want smaller files that don't eat up all your
space.  If you are not careful, you will end up with 40+ TBs like some folks
here have.
