Make sure you turned on ringtone syncing for the phone - I think mine was off by default when I last wiped my phone. When the phone is plugged in, you need to go to phone settings and check the sync ringtones option.
You'll need to log into the store using the same account you originally used to download the app as its tied to the account. Every app you download (free or paid, doesn't matter) is tied to your iTunes account. You can use multiple accounts to get apps, but then the computer must be authorized for all of the accounts (by logging in using the account as you're prompted).
