Done.  Here's the log.

 - Use [EMAIL PROTECTED] for emailAddress.
 - Use HCOOP, Inc. for O.
 - Use deleuze.hcoop.net for CN.
 - Modified /usr/sbin/mkpo3dcert and /usr/sbin/mkimapdcert to make the
   keys expire in 10 years rather than the default of 1 year.
 - Generated new keys with these parameters.
 - Require STARTTLS as per
   http://edseek.com/~jasonb/articles/exim4_courier/courierimap.html
   and disable the alternate port.  Did this for both IMAP and POP3.
