Sorry forgot to address this question. One common admin mistake is simply 
careless file deletion. The current HDFS Trash should protect this, but 
empirically this is often noticed from sudden drop of block counts or total 
space usage, which the admin team usually closely monitors (in contrast, file 
count is typically less watched).
