I have created a somewhat generic model to create targets (entities)
automatically, depending on the party attribute.
At this point, the GH will create the patient, but we can include health
professionals too.
Institutions could also be generate, but because the institution code is
required today, we would need to pass it as a param (or generate a default
code automatically).
