What is 'design_file'? Does it expect a START_FULL before LIBRARY? Is 'switch' invoked anywhere else? Do you have any 'go to state 0' output in your debug file? Does START_FULL appear anywhere else in your grammar? Which 'modification' are you removing? Both, or just the start rule?
