Not sure if I get it all right, but fetching a repository source, is
unfortunately case insensitive in repository 1. So if the document is
called Test.xml, you can fetch it with 'test.xml' as well as with
'Test.xml'. Make sure though that you have the latest versions of all
hippo cocoon extension jars, because invalidation of source used to fail
when they would be fetched with for example 'Test.xml' and the actual
source was 'test.xml'. This is fixed recently, but cannot remember which
jar it was...so take all latest versnios, you are good anyway :-))

You could use maven to override the file before the compile
phase, so
you can implement your own naming convention if you want, but I
personally prefer lowercase urls. As far as I know there is no real
SEO guideline on url cases.
