Cold ischemic time must be documented as well. This is important when the tissue goes for xray and is not immediately placed in formalin. Also, the limit for time in formalin is 72 hours, so you need to document - even if more than 72 hours. For us 3pm is the cutoff for breast tissue to get 6 hours fixation.
