In my experience and opinion, grossing and embedding are the main sources of extraneous tissues ("floaters"). This issue is discussed in detail in the section "Root Cause Analysis for floaters prevention during grossing and embedding in surgical pathology histology laboratory" in my book Grossing Technology pp. 254-265.
