I've being suffering this bug since long time ago, I was thinking it was a very specific issue, so posting it probably won't solve anything. I wasn't finding any similar issues neither.
Recently I detected the same problem in "Get a life mod", so maybe this isn't as rare as I thought. Basically you change screen resolution, or other video settings, then during map load, when game is initializing map entities, it crashes.
