It's a known error with HLSW that it has the potential to erase your ban
lists. Thus, on our servers, we never, ever, use the Ban List tab, but
ALWAYS enter bans in console, in the same way that you would in-game.
"banid N STEAM_0:1:234567 kick; writeid" "addip N 123.4.56.789; writeip".
In addition, we keep our banlists backed up and exec them at the start of
each map.

I may be wrong here, but I believe that if you find that you've accidentally
wiped your lists with HLSW, you can restore your ban lists by doing the
following: goto the console and type "exec banned_user; exec banned_ip" in
console, and it will execute both your banned_user.cfg and your
banned_ip.cfg.

Suck.

I had exec banned.cfg, but not writeid. I am indeed using HLSW, and may have
pressed the reload button (more then once), I'll keep an eye on this.

If they are reversed, it will erase your banned file. Also, be careful
