I'm the server provider, and each install is a clean install using the
hldsupdatetool.

i never have this problem.
this problem usually occurs when a server providor makes a "copy" of an
already downloaded server.
the best way to avoid this problem is to download a dedicated server
rather
than copying one .

One way is if someone uses HLSW and doesnt update the Bans list before
Adding a ban and saving it. (Particularly happens on out of date
versions)
