It happened on Windows XP SP3 and now the same on the clean installation of Vista 32-bit. The same happened when I used no MetaMod and SourceMod, only Mani Admin plug-in, so it's unlikely to be related to some particular plug-in. 32 slots, tickrate doesn't matter, occurred with both 66 and 100. No other modifications, just pure servers installed via HldsUpdateTool.
