I've noticed it only happens when I've just changed maps and there is only a few people on the server less than 5. Once more people show up it shows up about 10 minutes into the map... For some on the server it never shows up again, and have to exit to come back into the server again. It's random for me however those are the times I've noticed it happens the most.
