We are having this problem also in Australia. Two box's in two separate
locations in Australia both with the same problem, no configuration changes
since.. One can only assume it's valve/steam. I wish we could hear back
soon. It's effecting business.

Same here in Germany (Debian Etch 64 on an Intel Core2Duo).

Are you waiting before the first two are completely in the game before
trying to add more? There is a hard coded limit on players connecting from
the same IP, but once they are in the game more can come.

If you want the bans to stick you have to re-exec the ban files each
mapchange...sometimes. It seems to depend. I'm not sure what exactly it
depends on, but give it a shot.

Add to server.cfg:

exec banned_user.cfg
exec banned_ip.cfg

I'm working at a game publisher in Germany and we have Community 
Management AND we have an sysadmin emergency service (every weekend 
someone has to suffer *muha*) ;)

I can't belive that such a company like valve doesn't get it managed to 
setup such basic things.
