Are any of you setting the maxplayers cvar? This was a bit of a trick kinda employed in l4d(1) when people wanted to restrict a server to coop. Evil trick, but it sorta worked. Obviously it's not needed in l4d2, with the sv_gametypes flag... course, we do only have coop sofar with the demo.
