After reading this message from Zoid, i came to conclusion that any 
campaign that uses any of the original survivor model files (like 
something ported from L4D1 -> L4D2), will cause crash if passing is 
loaded. That in mind, the culprit that seems to taken our servers down 
was no_mercy_2.vpk and i have removed it completely from servers so if 
you use it, remove it too to make your servers work fine again with the 
passing campaign.

I have no idea about will it occur on versus mode because i'm kind of 
alone now and no one else with me to test it.
