Just a quick update. I isolated this to my web-server, but setting "sv_downloadurl" to empty quotes and effectively disabling the fast-download feature. No, does not appear to be something with TF2, SRCDS or SM. It's something with my Apache configuration or underlying file-system and similar symbolic links there.
