In looking at your jpg of the layout, it looks pretty good, nice and logical, well presented.
I am assuming that you are including the information at the bottom part of the form in a different table than the top?
There really would be, in a relational database, a one-to-many relationship, that is to say that any author/contributor (what you have called "Name") could be the author/contributor on more than one document (what you have called "Title") and so for an efficient and optimized database, the fields relating to the "Name"d person (Emails - primary & secondary and ICQ) would be stored in a separate table.
Additionally, some/many documents have multiple authors, so perhaps additional author/name fields could be added, again linking to their own contact record information.
I love the way you have the logo there. Again, the form looks really nice.
Good luck with your database. I've stopped work on the ones i have been doing for projex. No sense two of us doing the same work...
Speaking honestly, I would have liked for us to work with the one i had started and customize it to suit our purposes... That would be a collaborative effort amongst all of us, like with the collective wiki or blog - Kate built a blog and we used it, people didn't build a separate one, they used hers.
