I'm chiming in late to this. This has only happened to me once - with a  
mouthpiece anyway. Pipes on the other hand...
 
I have a few pairs of vice grips and a bag of removable rubber tips.  
They're always helpful for loosening plumbing fixtures where you do not want to 
 
dent or mar the finish. 
 
One pair on the rim and another on the shank with the protective pads  
usually does the trick. It also does the trick on pipe fittings that have 
rusted 
 shut.

103 and just for kicks I grabbed the shank hard while it was  seated in the 
horn and gave the rim a good twist, and lo and behold the screw  rim came 
loose and I got it off! This also worked on my Shilke screw rim.  
So here is another solution.
