Sorry I have been meaning to reply for awhile, but just couldn't seem to get 
around to it.

Here is my two cents:

Many years ago in a conversation with John Barrows he said this:

  You could have the most perfect embouchure in the world and bad breath 
control and you couldn't play *&^*.

  Notice I left out the last word

  But you could have really good breath control and you could play our your 
nose.

Moral being:  If I were you I would look to my breath instead of embouchure.  I 
know it is always my problem when I come back after a short or a long layoff.

I'm not sure what's happening to you, but it may be an inefficient
embouchure issue.  I had very poor range & endurance for several years due
to an inefficient embouchure.  I found a program designed for trumpet players
that remedied my chop problems, a development system described in a book,
"The Balanced Embouchure", by Jeff Smiley.  This unique development
system made it possible for me to achieve excellent range and endurance. 

This book is not available in music stores. In the USA, you can only get it
directly from the author himself. In Europe it's available through a couple
