I've been having the same problem since I've updated some packages on my
machine. It used to work perfectly before. The printer/scanner is an HP
Officejet 7210. The scanimage commandline is:
   scanimage -p -x 210 -y 294 --compression none --mode color --resolution
300 -b --batch-scan=yes

After some investigation I could reproduce this misbehavior on
hplip-0.9.7-r3 hplip-1.6.6a and hplib-1.6.7 (these are gentoo ebuild
versions but should correspond to the hplip package versions).

The version that probably worked is hplip-0.9.8-r2 unfortunately the ebuild
is no longer in the portage tree. 

Apart from not seeing the out-of-paper, the scanner also stops about at 75%
of the page and starts scanning inches as pages. So that's about 2 cm of
scanning and then it creates a new output file.
