I haven't the faintest what EmptyReference is--it's certainly not an
error message from any of the code I've written.
The "no header" message is from the HTTP code and indicates that the
request did not receive a header. There's a bug related to this that
should be fixed in the current snapshot. (If a document returned an
error and persistent connections are on, then all requests to that
server would return "no header" after that point.)
