The maximum number of stars is set by an attribute:
<http://www.htdig.org/attrs.html#max_stars> which defaults to 4. As to why
you do not see pages with 2 or 3 stars, it depends on the scoring factors
and how the webpages are designed. On some websites, the backlink_factor
may need tuning--it's set fairly high to favor pages that have a high
number of pages pointing to them.

The number of stars and $(PERCENT) are related--the highest score is given
the maximum number of stars and a 100% rating. Then pages are rated
relative to the score of that particular page.
