I've been experimenting with the number of entries in the list, using the `file` mechanism. It seems to be fine up to about 7000 entries (I was wrong about there being about 5000). Once we reach around 7194 entries or 208169 bytes behaviour becomes increasingly eratic. Increasing numbers of URLs are incorrectly matched and rejected. By 8000 entries everything is rejected. Using the include: mechanism, I couldn't get it to work at all until I terminated the last line with a line break! After that it seemed to behave similarly to using the `file` mechanism.
